A Clinical Study of Abdominal Trauma


Abstract
This report is a clinical study of 94 cases of traumatic abdominal injuries treated in Won Kwang University Hospital from Jan. 1980 to Jan. 1983.
The results were obtained as follows;
1) The aex distribution was 91 males and 3 females with the ratio of 30: 1. The age prddominance was in second decades (40.4%). Blunt traumas were 68 cases, and penetrating traumas were 26 cases.
2) The injured organs were small intestine (35.9%), liver (12.5%), spleen (7.8%), retroperitoneum (7.8%), kidney (6.3%), stomach (6.3%). Among blunt injuries, small intestine was the most predominant (30%), but among penetrating injuries, the stomach was the mostcommonly involved organ (21.9%).
3) The seasonal distribution was predominant in winter (33%).
4) The duration from trauma to operation was predominant in over-12hrs (33%).
5) The mode of trauma was the most frequent in traffic accident (28.7%) and violence (28.7%).
6) The clinical manifestatons were abdominal pain (97.9%), abdominal tenderness (95.7%), diminished peristalsis (87.2%), abdominal rigidity (72.3%), rebound tenderness (72. 3%).
7) In erect abdominal X-ray, subphrenic free air was visualized in 14 cases among 56 cases (25%). It was 33% in the colon, 25% in the stomach, 23.3% in the small intestine in order.
8) The abdominal paracenthesis at emergency room showed positive in 96.3% overally.
9) The operation was performed in the greater part of patients without use of blood.
10) The associated extraabdominal injuries were chest injuries (including rib fractures> (29%), pelvic bone fracture (19.4%), low extremity fracture (19.4%) in order.
11) The methods of operations were variable.
12) The incidence of postoperative complications was 43.6%. Among them, wound infection was the most common complication (12.2%), intestinal obstraction (12.2%) in order.
13) The causes of death were sepsis (3 cases, 42.9%), superimposed brain injuries(2 cases,. 28.6%), multiple organ failure (1 case, 14.3%) and hypovolemic shock (1 case, 14.3%).
14) The mortality rate was 10.6% in blunt trauma, and no mortalily presented in pene-strating.
